Membership Has Its Privileges.....

The Southwest Laborers' District Council, in coordination with the LiUNA Midwest Region and affiliated Local Unions, provides Union members with a $2,000 Basic Life Insurance policy as well as a $2,000 Accidental Death and Dismemberment policy payable through MefLife.

What is the cost of this benefit?  There is no additional cost to members.

Who is eligible for this coverage?  This benefit is for all union members in good standing, whether you are an active member or a Gold or retired member.

Do I need to elect a beneficiary?  Yes, you should designate a beneficiary so that the benefits are paid based upon your wishes.

Beneficiary designation forms are available at your Local Union office or you can download a beneficiary form here.  Just CLICK HERE, print it and then fill it out and send a signed original to your affiliated Local Union (keeping a copy for your records). 

If there is no beneficiary designated or no surviving designated beneficiary at your death, MetLife may determine the beneficiary to be one or more of the following who survive you; your spouse, your child(ren), your parents, or your siblings.

How should a claim be filed?  All claims should be filed through your Local Union office.  Each Local Union will have the appropriate forms on hand.
